Rheumatoid Arthritis Presented by Chronic Follicular Bronchiolitis: A Rare Case Report

DOI : 10.5146/jcpath.2019.59 Aim: Rheumatoid arthritis (RA) is a systemic inflammatory disease and pulmonary involvement is common. Patients presenting with pulmonary involvement before the diagnosis of rheumatoid arthritis are important due to rarity.

Case: A 40-year-old female patient visited chest diseases department of our hospital because of shortness of breath and cough. High resolution computed tomography (HRCT) revealed peribronchial thickening, tree-in-bud pattern, ground glass opacities and nodular densities. Histopathologic sections showed lymphocytic inflamation of the pleura and hyperplastic lymphoid follicles with reactive germinal centers within bronchiole walls. These histopathological features evaluated in favor of chronic follicular bronchiolitis.

Conclusion: Manifestations of lung disease in rheumatoid arthritis typically follow the development of articular disease, but in some instances lung involvement is the first manifestation of rheumatoid arthritis. Therefore, pulmonary involvement of rheumatoid arthritis should be considered in patients with pleural involvement and chronic follicular bronchiolitis.
