INTRODUCTION: We present a case of endometrial stromal nodule(ESN) with smooth muscle differentiation , a rare entity that causes diagnostic difficulties.

CASE REPORT: A 62 year old female patient who complained of bleeding for 2 years and discomfort was referred to surgery. Grossly there was a yellowish polypoid mass measuring 6,5x6x5,5 cm in the uterine cavity. Microscopically the tumor was characterized by two different cell types which intertwined each other. One of them is small uniform cells with round or oval nuclei resembling normal proliferative phase endometrial stroma and was positive with CD10. The other type is spindle shaped with fusiform blunt-ended nuclei like smooth muscle cells and was positive with caldesmon and SMA.

CONCLUSION: ESN with smooth muscle differentiation is a rare benign mesenchymal tomur that needs to be distinguished from tumors with similar morphologic features. And also there is no reliable pre-operative diagnostic procedure.
