Amaç: Bu olgu sunumunun amacı, meningotelyal benzeri girdapsı yapılar gösteren dediferansiye liposarkomu tanımlamak ve ayırıcı tanıda
önemli sorunlar yaratabileceğine dikkat çekmektir.
Olgu: Abdominal ağrı, halsizlik gibi spesifik olmayan semptomlarla başvuran 46 yaşında erkek hastada bilgisayarlı tomografi ve ultrasonografi
ile retroperitoneal kitle saptandı. Eksize edilen tümör 16x10x8 cm ölçülerinde nodüler, grimsi, sert, düzgün sınırlı ve düzgün yüzeyliydi.
Literatürde bildirilen az sayıda retroperitoneal meningotelyal benzeri girdapsı yapılar gösteren dedifferansiye liposarkom olgusu bulunmaktadır.
Histolojik ve klinik olarak ayırıcı tanıda pleomorfik sarkom ve miksofibrosarkom yer alır.
Sonuçlar: Meningotelyal benzeri girdapsı yapı gösteren liposarkom sıklıkla lokal nüks ve uzak metastazla seyreder. Tedavide tam cerrahi
eksizyon tercih edilir ve adjuvan kemoterapi veya radyoterapi gerekebilir.
Aim: The aim of this case report is to define dedifferentiated liposarcoma with meningothelial-like whorls pattern and point to the possible
significant problems in the differential diagnosis.
Case: A 46-years-old man presented with nonspecific systemic symptoms, abdominal pain and weakness and a retroperitoneal mass was
diagnosed by multislice computed tomography and ultrasonography. The mass excised was 16x10x8 cm in dimensions with nodular, grayish,
firm, well-localized, well-demarcated properties. There are few dedifferentiated liposarcoma with meningothelial-like whorl pattern cases of the
retroperitoneum reported in the literature. Histological and clinical differential diagnosis of dedifferentiated liposarcoma with meningothelial-like
whorling includes pleomorphic sarcoma and myxofibrosarcoma.
Conclusions: Liposarcoma with meningothelial-like whorl is associated with a significantly high local recurrence and distant metastasis rate.
Preferred treatment is complete surgical excision and adjuvant chemotherapy or radiation therapy may be necessary.
