Diagnostic and Prognostic Value of E-Cadherin and CD10 Expression in Endometrial Intraepithelial Neoplasia and Endometrioid-Type Endometrial Carcinomas

DOI : 10.5146/jcpath.2026.687 Objective: Diagnostic challenges may occasionally arise in distinguishing endometrial intraepithelial neoplasia (EIN) from FIGO grade 1 endometrioid endometrial carcinoma. In this study, we aimed to evaluate the distribution of E-cadherin and CD10 expression in EIN, grade 1 endometrioid endometrial carcinoma, and higher-grade endometrial carcinomas, and to assess their contribution to differential diagnosis as well as their potential prognostic value. Material and Methods: A total of 71 hysterectomy specimens diagnosed at Konya Training and Research Hospital were included in the study. Of these, 15 cases were EIN and 56 were endometrioid-type endometrial carcinomas. Among the carcinoma cases, 20 were classified as FIGO grade 1, 20 as FIGO grade 2, and 16 as FIGO grade 3. Immunohistochemical staining for E-cadherin and CD10 was performed in all cases. Expression levels were evaluated using a histoscore method based on staining intensity and the percentage of positive cells. Statistical analyses were conducted using Chi-square, Mann–Whitney U, and Kruskal–Wallis tests, with a p-value less than 0.05 considered statistically significant. Results: E-cadherin expression in EIN cases was observed to be homogeneous and membranous, whereas carcinoma cases showed a heterogeneous distribution with membranous-cytoplasmic staining (p<0.001). In endometrial carcinomas, both E-cadherin and CD10 expression significantly decreased with increasing tumor grade (p<0.01 and p<0.05, respectively). Additionally, low E-cadherin expression in carcinoma cases was associated with deep myometrial invasion and advanced FIGO stage. Stromal CD10 expression was strong in EIN cases but decreased in carcinoma cases, showing an inverse correlation with higher tumor grade. Furthermore, a significant positive correlation was found between E-cadherin and CD10 expression (p<0.001). Conclusion: Loss of membranous E-cadherin expression and decreased stromal CD10 expression may represent important immunohistochemical alterations associated with malignant transformation and tumor progression in endometrial lesions. The combined evaluation of E-cadherin and CD10 may contribute to the differential diagnosis, particularly between EIN and low-grade endometrioid endometrial carcinoma, and may aid in predicting tumor biology.
