Case: A 46-years-old man presented with nonspecific systemic symptoms, abdominal pain and weakness and a retroperitoneal mass was diagnosed by multislice computed tomography and ultrasonography. The mass excised was 16x10x8 cm in dimensions with nodular, grayish, firm, well-localized, well-demarcated properties. There are few dedifferentiated liposarcoma with meningothelial-like whorl pattern cases of the retroperitoneum reported in the literature. Histological and clinical differential diagnosis of dedifferentiated liposarcoma with meningothelial-like whorling includes pleomorphic sarcoma and myxofibrosarcoma.

Conclusions: Liposarcoma with meningothelial-like whorl is associated with a significantly high local recurrence and distant metastasis rate. Preferred treatment is complete surgical excision and adjuvant chemotherapy or radiation therapy may be necessary.
