  Introduction: Chondrolipoma is composed of mature adipocytes and varying amounts of cartilaginous tissue. This benign mesenchymal tumor is very rare and cartilaginous metaplasia at long-lasting lipomas is a known etiology. Chondrolipoma described in the skeletal system, breast, pharynx, and nasopharynx so far. We report the lesion at the thenar region on the left hand. The surgical specimens were formalin-fixed and paraffin-embedded after decalcification procedure. The section was stained with routinary H&E.

Case Report: A 55-year-old woman suffered from swallowing on her left hand. The surgeon removed the lesion and sent us with prediagnosis such as liposarcoma, angiosarcoma, lipofibroma. Macroscopically lesion has 3 cm larger dimension, well-circumscribed capsule, and firm, grey-white irregular shape area surrounding with homogenous yellowish soft tissue on the cut surface. Cartilaginous nodules and ossifying areas within lipomas are diagnostical features for us.

Conclusion: Awareness for ossifying chondrolipoma on the hand, differentiating from more common diagnosis chondroid lipoma or malignant mesenchymal tumors are the main goal.
