Basal cell carcinoma (BCC) is most frequently seen skin cancer which occur predominantly in sun-exposed areas. Although it remains the most common cutaneous malignancy, its occurrence in the perianal region is very rare. We present a patient with tumoral mass diagnosed as perianal BCC.

The surgical specimens were formalin-fixed and paraffin embedded. The section were stained with routinary H&E. Immunohistochemistry was performed.

A 70-year-old male presented with a bleeding perianal mass that had been growing for the last 1 years. Macroscopic examination revealed an exophytic condyloma-like nodular lesion approximately 2.5 × 2.5 cm in size with surface ulceration in the left lateral perianal region. Histopathological and immunhistochemical results were compatible nodular BCC.

Basal cell carcinoma (BCC) is a common skin tumor that constitutes 75% of nonmelanocytic skin tumors. It is quite rare in areas with no ultraviolet exposure, especially the genital and perianal regions. Condyloma acuminatum, Paget’s disease and squamous cell carcinoma, solitary giant trichoepithelioma should be kept in mind in differential diagnosis of perianal BCC.

Perianal BCC cases are diagnosed later than other types and can be mistakenly treated as inflammatory or infectious lesions. Histopathological evaluation of perianal region lesions is therefore quite important.
