Nonparasitic cysts are divided into two categories as primary (true, epithelial) and secondary (pseudocysts, nonepithelial) according to the presence of epithelium. Epidermoid cysts with squamous epithelium are predominantly seen in children or young adults.

Case: A 12-year-old male patient presented to emergency room with a complaint of abdominal pain and splenomegaly was noticed on physical examination. A splenectomy was performed with a hydatid cyst diagnosis. Unilocular cyst with a diameter of 8 cm was observed on the cross section of the spleen. Morphological and immunohistochemical findings of the case revealed a "primary epithelial cyst in the spleen / epidermoid cyst".
Conclusion: Although splenic epithelial cysts are rare, they should be kept in mind in the differential diagnosis of cystic formations.
