The Effect of Signet Ring Cell Component on Prognosis in Colorectal Musinous Adenocarcinomas

DOI : 10.5146/jcpath.2020.69 Aim: Mucinous adenocarcinoma (MAC) has poor prognosis than classic adenocarcinoma but has better prognosis than signet ring cell (SRC) carcinoma. However, the effect of signet ring cell component in MAC on prognosis is still unclear. In this study we aim to evaluate the effect of SRC component in mucinous tumour and in lymph node metastasis on prognosis.

Materials and Methods: 40 patients with MAC diagnosis and follow up between 2011 to 2013 were included in study. The slides of the patients are re-evaluated and percent of SRC in mucinous tumour and presence of SRC in lymph node metastasis assessed. In Roc analysis, at T4 stage, the cut of value SRC percentage was calculated as 7% with 53.8% sensitivity and 50% specifity. Based on this results, the effect of SRC presence on prognostic values, disease free survival (DFS) and overall survival (OS) was evaluated.

Results: The mean DFS was 26 and, OS was 27±26 (min:1-max82) months. 21(52.5%) patients had ? 7% and 19 (47.5%) patients had >7% SRC ratio. There was no relation between SRC ratio and OS (p=0.120) and DFS (p=0.269). However, SRC presence in the lymph node had significantly related to OS (p=0.006) and DFS (p=0.001). There was a significant relation between SRC ratio and stage in tumour. And SRC presence in lymph node was also significantly related with perineural invasion, lymphovascular invasion and mortality rates of patients.

Conclusion: SRC ratio was related to stage of colorectal MAC, but in terms of prognosis, it was not related to OS and DFS. However, presence of SRC in lymph node was related to low OS and DFS, and associated with poor prognosis.
