Introduction: Meningiomas are common benign neoplasms of the central nervus system. They have long term curability if they are excised completely.Anaplastic meningioma is a rare neoplasm. It has high recurrence than low-grade meningiomas.
Case Report: The patient was a 65-year-old famale, who developed progressive gait instability. Cranial magnetic resononce imaging revealed a 4x7,5 cm mass in the posterior fossa; inferior part of tumor invaded into the C2 vertebrae posterior. She under went occipital decompressive craniectomy and complete excision of the mass. Histological specimen demonstrated malign cytological features resembling carcinoma and spindle tumoral cells that are parallel, mixed, storiform pattern in a focal area of tumor. After immunohistochemical staining for differential diagnosis, the case was reported as anaplastic and fibrous meningioma.
Conclusion: Anaplastic meningioma arises de novo or as a result of the progressive transformation of a low-grade meningioma. It keeps unclear whether or not all low-grade meningiomas pass trough stage of atypical meningioma in the progression to anaplastic meningioma. The coexistence of anaplastic and low-grade component has been rarely reported.
