The Relationship Between HER-2 Positivity and Histopathological Findings in Gastric Carcinomas

DOI : 10.5146/jcpath.2018.32 Aim: HER-2 (Human epidermal growth factor-2) gene amplification is seen in 7-34% of gastric carcinomas. It is more frequently observed in observed in tumors with proximal location and intestinal type morphology. The aim of this study was to determine the prevalence of HER-2 in gastric carcinomas, to evaluate HER-2 expression in comparison with immunohistochemical (IHC) and silver in situ hybridization (SISH) methods and to demonstrate its association with histopathological parameters.

Materials and Methods: 203 cases diagnosed as gastric cancer between 1996-2016 were included in the study. Patient"s age and sex, location and size of the tumor, histological type, depth of invasion, perineural invasion, lymphovascular embolism, lymph node metastasis, local invasion or distant metastasis were recorded. Evaluation of the HER-2 IHC score was 0, +1, +2, +3 and the scores +2 and +3 were accepted as positive. In these cases, amplification was investigated by applying dual silver in situ hybridization/immunohistochemistry (SISH/IHC) method.

Results: 19 of cases have IHC +3 positive and 20 of cases have +2 positive results. HER-2 amplification was detected in 14 of 20 (70%) patients with IHC score +2 and 18 of 19 (94.7%) patients with IHC score +3. 22 of 32 cases were male and 19 of 32 cases were <65 years old. 9 of cases were at proximal, 22 was at distal while one tumor was linitis plastica. According to the Lauren classification, 27 were intestinal, 3 were diffuse and 2 were mixed type. Perineural invasion was detected in 20 cases, lymphovascular invasion in 26 cases and lymph node metastasis in 27 cases. According to the TNM staging, 19 of the HER-2 positive cases were in T4a, 5 were T3, 4 were T2, 3 were T1b, 1 was T1a.

Conclusion: HER-2 gene amplification was 19.2% in the study. In concordant with the literature, it was statistically significant in tumors showing intestinal type morphology. Anahtar Kelimeler : Gastric carcinoma, HER-2, SISH/IHC, Gastrectomy
